IMMIGRATION MEDICAL EXAMINATION 

I-693 FORM

QUALIFIED MEDICAL CLINIC

The doctor is a Civil Surgeon for the United States Citizenship and Immigration Services (USCIS).  This medical clinic is cosmopolitan.

REQUIREMENTS / FEES / GOOD FAITH ESTIMATE

IMMIGRATION MEDICAL EXAMINATION CONSULTATION: $300

TESTS: Tuberculosis (QuantiFERON or T-Spot) $190, Syphilis $23, Gonorrhea $64, Blood Draw $15

(if no documentation, you have the option to verify immunity: HepB $60, MMR antibody titer $110, Var antibody titer $56)

VACCINATIONS: TDaP (booster every 10 years) $62, MMR (if no proof of immunization or immunity) $111, VAR (if no history of vaccination or disease) $198, flu (if season: 1 October - 31 March), COVID (pharmacy $0), HepB (if age <60) $87, PCV20 (if =age65 and no history of any pneumococcal vaccination): $421

The Immigration Medical Examination Consultation Fee of $300 is for the initial visit and any follow up visit fees for immigration or completion of the form are  at no additional charge.

We charge upfront.  We always provide receipts, including for employer reimbursement and tax preparation. 

INSURANCE

For contracted PPO health insurance plans only, we charge upfront for the exam, the lab will bill the insurance for the testing, and we bill the insurance for the exam and any immunization; then what the insurance pays us for the exam we will refund back to you.  If you wish to buy health insurance consult an Insurance Agent.

INVESTMENT

The medical examination is a one time investment to immigrate to the United States of America.

  • marriage license / name change documents (if any)

TESTING/VACCINATION

  • Reports of above Requirements (if any) (don't worry if you haven't had any of this testing done yet, we will help you)

  • Reports for your other diseases (if any)

  • For outside tuberculosis report, please bring documentation specifying whether the test is QuantiFERON or T-Spot because these blood tests are the only two which are approved by USCIS.

FORMS

We have the latest I-693 form (electronic format, 14 page document, Edition 9 Mar 2023 expires 31 Jul 2025).  Upon completion of all tests and vaccinations, we will ask you to return for a follow-up visit to review and sign your form.  You will be given the completed form in a sealed envelope (do not open) for you to submit to USCIS.  You will also be given a copy of the form for your records.  As per I-693 instructions, we follow U.S. Department of Health and Human Services (HHS) regulations and Centers for Disease Control and Prevention (CDC) guidelines.

TIMELINE

It is most expeditious to submit I-485 together with I-693.  If you can not submit them together then the next best thing is to submit the I-693 within 60 days of submitting the I-485.  Although I-693 is valid for 2 years from the civil surgeon's signature date, the Edition of the form can retire prior to the expiration date on the form so waiting longer may trigger an additional Request For Evidence (RFE) having to re-do the form.

IF YOUR TUBERCULOSIS TEST IS ABNORMAL

Tuberculosis is an infection that primarily infects the lungs, but can spread anywhere. It is not uncommon in the rest of the world; 2 billion people in the world have Tuberculosis infection so America screens all immigrants and requires treatment if necessary. If your tuberculosis blood test is abnormal, you will need to get a chest X-ray and start treatment (Rx).  In >99% of the cases we see, the chest X-ray is normal.  87% of active tuberculosis cases are due to persons progressing from latent (inactive) tuberculosis.

= LATENT (INACTIVE) TUBERCULOSIS

CHEST X-RAY ABNORMAL

= ACTIVE TUBERCULOSIS
