INFORMATION
USCIS instructs us to follow U.S. Department of Health and Human Services (HHS) regulations and Centers for Disease Control and Prevention (CDC) guidelines. [I-693 Instructions p.4] You do not have to read the instructions or bring in the I-693 form because we have all the information. In addition to the below testing and immunizations, the medical history and physical examination includes a mental and physical examination.

The Immigration Medical Examination Consultation Fee of $300 is for the initial visit. Follow up visit fees for immigration or completion of the form are at no additional charge.

BILLING
ACCOUNTING
We charge all applicants upfront. We always provide receipts, including for employer reimbursement and tax preparation.
INSURANCE
For PPO (not HMO) health plans, with contracted health insurance, we charge upfront for the exam, bill the insurance for the exam and any immunization (the lab will bill your insurance directly); then whatever the insurance pays us for the exam we will refund back to you. If you wish to buy health insurance, you may consult an Insurance Agent.
INVESTMENT
To immigrate to the Unites States of America, medical examination is a ONE time investment. USCIS will not charge you to submit this form.
FORMS
We have the latest I-693 form (electronic format, 14 page document, Edition 9Mar2023 expires 31Mar2025). Upon completion of all tests and vaccinations, we will ask you to return for a follow-up visit to review and sign your form. You will be given the completed form in a sealed envelope (do not open) for you to submit to USCIS. You will also be given a copy of the form for your records. We recommend you mail the form to USCIS with tracking for the rare <1% chance it gets lost you have proof of submission.
TIMELINE
It is most expeditious to submit I-485 together with I-693. Effective April 4, 2024, any Form I-693 that was properly completed and signed by a civil surgeon on or after Nov. 1, 2023, does not expire, and can be used indefinitely as evidence to show that the applicant is not inadmissible on health-related grounds.
IF YOUR TUBERCULOSIS TEST IS ABNORMAL
Tuberculosis is an infection that primarily infects the lungs, but can spread anywhere. It is not uncommon in the rest of the world; 2 billion people in the world have Tuberculosis infection so America screens all immigrants and requires treatment if necessary. If your tuberculosis blood test is abnormal, you will need to get a chest X-ray and start treatment (Rx). In >99% of the cases we see, the chest X-ray is normal. 87% of active tuberculosis cases are due to persons progressing from latent (inactive) tuberculosis.
