Report of Immigration Medical Examination and Vaccination Record

  • USA Employment Authorization Card (if any)

  • Alien # (if any)

  • name change documents (if any, including marriage license)

  • USCIS online account # (if any, 13 characters: three letters followed by then numbers)

  • Reports of above Requirements (if any) (don't worry if you haven't had any of this testing done yet, we will help you)

INFORMATION

USCIS instructs USCIS Civil Surgeons to "follow U.S. Department of Health and Human Services (HHS) regulations and Centers for Disease Control and Prevention (CDC) guidelines."  [I-693 Instructions p.4  In addition to the below testing and immunizations, the medical history and physical examination includes a mental and physical examination.  The mental status examination includes cooperativeness.  "Physical disorders are diagnosed according to the diagnostic criteria in the most recent edition of the World Health Organization’s Manual of the International Classification of Diseases, Injuries, and Causes of Death (ICD) or by another authoritative source as determined by the CDC director.  The civil surgeon will ask you general questions during the medical examination to determine whether you have such a condition.  The civil surgeon may refer you to a specialist for further evaluation, if necessary."  The form requests "diagnosis, likelihood of recurrence of the harmful behavior, therapy given, and any counseling or referrals".

GOOD FAITH FEE ESTIMATE

IMMIGRATION MEDICAL EXAMINATION INTIAL CONSULTATION: $300

The Immigration Medical Examination Consultation Fee of $300 is for the initial visit.  Follow up visit fees for immigration or completion of the form are at no additional charge.

TESTS

  • Age 2+: Tuberculosis: (QuantiFERON or T-Spot): $190 

  • Age 18-44: Syphilis: $23

  • Age 18-24: Gonorrhea: $64

  • Blood Draw: $15​

  • Immunity Blood Test (if you prefer, instead of providing proof of vaccination): HepB $60, MMR $74, Var $56

VACCINATIONS

   COVID: NOT required (as of 22 Jan 2025 regulation)

BILLING

ACCOUNTING

We charge everyone upfront.  We always provide receipts, including for employer reimbursement and tax preparation. 

INSURANCE

For contracted PPO (not HMO) health insurance, we charge upfront for the exam, bill the insurance for the exam and any immunization, the lab will bill your insurance directly.  Then whatever the insurance pays us for the exam we will refund back to you. 

Military and Kaiser doctors are not qualified to complete this form for you, however rest assured we are qualified and will give you tips on how to maximize your benefits using your existing health insurance.

NO INSURANCE

You may buy health insurance if you wish, however health insurance is not necessary.  This is a one time investment to immigrate to the United States of America.

FORM SUBMISSION

USCIS will not charge you to submit the medical form.

FORMS

We have the current edition of the Form I-693 (electronic format, 14 page document, Edition 9Mar2023. USCIS states:  "The current edition of the Form I-693 will be accepted until a new edition/extension of the Form I-693 is published. We do not have a date for when the new edition/extension of the Form I-693 will be published."  Upon completion of all tests and vaccinations, we will ask you to return for a follow-up visit to review and sign your form.  You will be given the completed form in a sealed envelope (do not open) for you to submit to USCIS.  You will also be given a copy of the form for your records.  We recommend you mail the form to USCIS with tracking for the rare <1% chance it gets lost you have proof of submission.

TIMELINE

Effective 2 Dec 2024, Form I-485 (Application to Register Permanent Residence or Adjust Status) must be submitted with Form I-693 (Report of Immigration Medical Examination and Vaccination Record).  Any Form I-693 that was properly completed and signed by a civil surgeon on or after Nov. 1, 2023, does not expire 

IF YOUR TUBERCULOSIS TEST IS ABNORMAL

Tuberculosis is an infection that primarily infects the lungs, but can spread anywhere. It is not uncommon in the rest of the world; 2 billion people in the world have Tuberculosis infection so America screens all immigrants and requires treatment if necessary. If your tuberculosis blood test is abnormal, you will need to get a chest X-ray and start treatment (Rx).  In >99% of the cases we see, the chest X-ray is normal.  87% of active tuberculosis cases are due to persons progressing from latent (inactive) tuberculosis so if your chest X-ray is normal you will still need treatment, but it will be much less then if your chest X-ray is abnormal.
